For the March-April issue of the Journal of Community and Supportive Oncology, the Editor in Chief, Dr David Henry, discusses two informative “how-to” articles, one on the implementation of a distress management program at an oncology hospital in Puerto Rico, the other on the prevention and treatment options for mTOR inhibitor-associated stomatitis. Dr Henry also shares his preferences for addressing health care reform, and he highlights a letter to the journal in response to the January-February issue Commentary on physician-assisted dying. Immunotherapies are at the fore again, this time with an insightful essay by Jane de Lartigue who writes that combination therapy is likely to be key in expanding the scope of immunotherapy into currently unresponsive patient populations, which raises questions about the optimal combinations and the timing and sequencing of combination immunotherapy. Three Original Reports span the clinical, supportive, and quality- and value-based care components of cancer care, with their respective foci on APF530 for nausea and vomiting prevention after cisplatin; patterns of care in whole-brain radiotherapy technique and delivery; and emergency department use by newly diagnosed cancer patients. As usual, there is a line-up of rare and challenging presentations in Case Reports on pulmonary sarcomatoid carcinoma presenting as a necrotizing cavitary lung lesions, palmoplantar exacerbation of psoriasis after nivolumab for lung cancer, primary cardiac prosthetic valve-associated lymphoma; and atraumatic splenic rupture as an initial presentation of chronic myelogenous leukemia.

Two Case Reports focus on gastrointestinal cancers, one in a patients with high-grade leiomyosarcoma of the transverse colon and who presented with bowel perforation, and another on 1,25-dihydroxyvitamin D hypercalcemia and imatinib hepatotoxicity in a patient with GIST, and the New Therapies feature provides a detailed examination of recent advances in managing gynecologic cancers.

In the June podcast for The Journal of Community and Supportive Oncology, Dr David Henry discusses the approval of alectinib as a new option for ALK-positive patients with NSCLC who have progressed on crizotinib, and a How We Do It article in which the authors the implementation of ipilimumab therapy in a private practice oncology group. The authors describe how they addressed start-up and reimbursement issues related to ipilimumab as a model for other expensive new cancer drugs as well. Also included are research articles on long-term community-based results of breast-conserving therapy in early-stage breast cancer, the use of the neurokinin 1 receptor antagonist in moderately emetogenic chemotherapy, risk assessment for hereditary breast and ovarian cancer in minority women, the impact of a nurse practitioner-led symptom clinic on emergency department use in cancer patients, and assessing outpatient oncology needs. Finally, Dr Henry examines evolving therapeutic strategies in melanoma.

For the May podcast for The Journal of Community and Supportive Oncology, Dr David Henry highlights original reports on the prognostic significance of HPV status in postoperative squamous cell carcinoma of the head and neck, the contributions of caregivers in American Indian cancer patients, and the omission of dexamethasone from antiemetics treatment in breast cancer patients with hepatitis B infection or diabetes. He also describes how providers at a comprehensive cancer center devised a framework for implementing survivorship care planning, and, in keeping with the ‘how we do it’ theme, he discusses an article by 2 practicing on oncologists on managing and treating multiple myeloma. The line-up is rounded off with articles on the effects of exercise interventions during different treatments in patients with breast cancer, the approval of trabectedin for some forms of advanced soft tissue sarcoma, and a Case Report on a rare case of hypoglycemia induced by a classical gastro-intestinal stromal tumor.

In his April podcast for The Journal of Community and Supportive Oncology, Editor-in-Chief Dr David Henry discusses Original Reports on olanzapine compared with fosaprepitant for the prevention of concurrent chemotherapy radiotherapy-induced nausea and vomiting and an analysis of patient-reported outcomes in prostate cancer after treatment with abiraterone acetate. Also among the line-up of Original Reports are studies looking at the diagnostic work-up for the detection of malnutrition in hospitalized cancer patients and at the use of video-conferencing to deliver cancer genetic counseling to high-risk underserved individuals. The Case Reports include an unusual case of non-small-cell lung cancer presenting as spontaneous cardiac tamponade, another of severe eosinophilia associated with cholangiocarcinoma, and a third on acute benzodiazepine toxicity exacerbated by concomitant oral olanzapine.
