At the time of this study, Dr. Poker was a pharmacy practice resident at the Erie VA Medical Center (EVAMC), Pennsylvania. She is now a clinical pharmacy specialist at Butler VA Medical Center, Pennsylvania. Dr. Chrymko is a clinical pharmacy specialist at EVAMC and a fellow of the American Society of Health-System Pharmacists. At the time of this study, Dr. Bostwick was an assistant professor of pharmacy practice at Lake Erie College of Osteopathic Medicine School of Pharmacy, Pennsylvania, and clinical pharmacy specialist in behavioral health at EVAMC. She is now a clinical pharmacist and clinical assistant professor at the University of Michigan Health System and College of Pharmacy, Ann Arbor.
